The Correspondence Unit processes routine requests for occurrence summary reports including:
- Collisions
- Reconstruction collisions
- Theft
- Property damage
- Witness statements
- Photographs
Fees
Required
- Occurrence Summary Report Request Form. Download the form here.
- Payment in full ($45.00).
- Government-issued photo ID (original or photocopy)
- Authorization to release (if you are not named in the report or are an involved party’s representative):
- Any form of authorization letter is accepted (dated within 12 months)
- Or use the provided Authorization to Release template
- Insurance companies may request on your behalf if named in the report or if they have your signed authorization letter.

Insurance companies, representing parties and law firms requesting routine disclosure of report(s) by mail
- Frequent requesters are encouraged to pre-purchase blank forms in bulk and mail in as needed.
- To order occurrence summary report forms:
- Specify the number of forms you wish to purchase
- Provide a contact name and mailing address.
- Enclose a cheque for the number of forms x $45.00 (including GST), payable to Calgary Police Service.
- Mail to the Correspondence Unit address above.
- If your company regularly submits individual reports, and we identify your requests are submitted on a regular basis, you will be asked to pre-purchase blank forms to support timely processing.
Additional information
Requests are processed in order they are received. Processing generally takes six to eight weeks.
If requesting urgent processing outside standard timelines, you must provide:
- A written correspondence explaining the urgency
- A completed occurrence summary report form.
Approved urgent requests will be processed within two business days.

- Wednesday: 10 a.m. to 3:30 p.m.
